Adult Name Changes
Filing Fee: $25.00 (additional certified copies are $4.00)
Forms are provided in the Probate Court Office.
- The Petitioner must be a legal resident of Autauga County and age 19 or older. If you are a legal resident of another county, you will need to contact the Probate Court in the county in which you reside for information on the procedure, as this process varies from county to county.
- The Petitioner must present picture identification, i.e., driver’s license or military identification. The Petitioner’s signature will be notarized in the Probate Court Office.
- In some cases, the Petitioner will need to provide a certified copy of his/her birth certificate and his/her social security card.

Minor Name Changes

Forms are provided in the Probate Court Office.
- Minor children must be legal residents of Autauga County.
- The Petitioner must present a certified copy of the child’s birth certificate. Certified copies may be obtained from the Autauga County Health Department at 334-361-3743 or the Center of Health Statistics at 334-206-5418.
- A person listed as the father on the birth certificate must petition jointly with the mother for a name change, or sign consent form. If no father is listed on the birth certificate of the father’s whereabouts are unknown you must publish notice to him in the newspaper.
- The Petitioner must present picture identification, i.e., driver’s license or military identification. The Petitioner’s signature will be notarized in the Probate Court Office.
- The minor child who is 14 or older must sign a consent form to have a legal name change.
